Co-occurring chronic conditions like diabetes, hypertension, and heart disease are abundant in the low-income communities served by Via Care Community Health Center (Via Care), where many individuals struggle to access essential, life-saving medications. This project expands our Medication Therapy Management (MTM) program to address access disparities by providing multifaceted support to chronically ill patients through one-on-one bilingual medication counseling; integrated pharmaceutical and primary care; and flexible telehealth follow-ups.
