The Empowering R.E.A.C.H. Foundation will provide over 2,000 Dignity Kits containing essential clothing and toiletries to low-income seniors and disabled adults discharged from hospitals in low-income neighborhoods in LA County. These kits restore dignity and support community reintegration for vulnerable individuals while also connecting them to critical housing resources and support services, preventing unnecessary institutionalization and homelessness.
