The Michael Hoefflin Foundation is a nonprofit 501(c)(3) organization dedicated to supporting children diagnosed with cancer and their families in the Santa Clarita Valley and surrounding communities. Since our founding, we have provided financial assistance, social connection, and emotional support to families navigating one of the most difficult journeys imaginable.
We are seeking a compassionate, organized, and bilingual Patient Family Liaison to join our team on a part-time basis. This role is central to ensuring that every family we serve feels seen, supported, and connected to the resources they need.
About this job
The Patient Family Liaison serves as the Foundation’s primary point of contact with families, helping to build trusting relationships and ensuring families are aware of and connected to available services. The ideal candidate will be empathetic, self-motivated, and detail-oriented. Strong communication, organizational, and computer skills are essential.
Roles & responsibilities
• Proactively maintain regular contact with all active families (at least once per quarter).
• Reach out annually to “infrequent” families within the MHF service area to encourage participation.
• Connect with new family referrals within one week, scheduling an introductory meeting to share MHF services and collect family information.
• Maintain accurate and up-to-date family records in the Outreach Information System, including services provided.
• Prepare and present monthly outreach reports to the MHF Board.
• Coordinate and attend major MHF Family Events such as the Harvest Party, Picnic, Holiday Party, and other gatherings as needed.
• Build and sustain relationships with hospitals, pediatric healthcare providers, community organizations, and faith groups to strengthen outreach and referral networks.
• Recruit, organize, and support volunteers for outreach activities and events
Qualifications
• Bilingual in English and Spanish (required)
• Strong interpersonal skills and a compassionate, family-centered approach
• Excellent organizational and computer skills
• Self-motivated and detail-oriented
• Experience working with families, community outreach, or nonprofits preferred
