What is your understanding of the issue that you are seeking to address?
As people enter their later years, they often face complex and deeply personal decisions about their health, finances, and long-term care. Many seniors and their families are unprepared for the realities of aging—lacking access to trustworthy guidance on retirement planning, funding options, and healthy aging strategies. Without this support, older adults are at risk of financial insecurity, declining health, and loss of independence. This initiative aims to close that gap by providing seniors and their families with the education, resources, and personalized support they need to make informed choices and live out their golden years with dignity, confidence, and peace of mind.
Describe the project, program, or initiative this grant will support to address the issue.
Smart Moves for Seniors and Caregivers is a comprehensive community education initiative by the 1Heart Cares Foundation, designed to empower older adults and their families to age in place with dignity, independence, and financial security. The program responds to the growing need for trusted guidance around retirement, healthcare, and well-being, offering seniors the tools they need to make informed decisions and maintain their quality of life.
The initiative features two core components:
Financial Education and Health & Wellness Workshops, covering key topics such as retirement budgeting, fraud prevention, healthcare costs, and available public resources —along with wellness support like health screenings, nutrition education, gentle fitness, and mental health resources.
Social Events and Community Outings aim to reduce isolation by fostering connection, engagement, and emotional well-being through meaningful shared experiences.
Describe how Los Angeles County will be different if your work is successful.
If 1HCF’s initiative is successful, Los Angeles County will become a more informed and supportive place for seniors. Older adults and their families—especially those facing financial and systemic barriers—will gain the tools and knowledge to make empowered decisions about health, finances, and long-term care.
Through workshops, personalized guidance, and accessible resources, seniors will be better equipped to avoid financial hardship, maintain their health, and plan proactively. More seniors will be able to age in place, preserving their independence and dignity in familiar communities.
The strain on emergency and hospital services will decrease as preventable crises are reduced. Families will feel more confident and less burdened, supported by clear information and a trusted resource network.
In the long term, this project will foster a healthier, more resilient aging population and help shift the narrative from vulnerability to strength, choice, and dignity in aging.
