
OFMP’s Helping Hands Outreach and Engagement Program
This grant will support OFMP’s Helping Hands Outreach and Engagement Program expansion of holistic, trauma-informed services for underrepresented and vulnerable populations in Los Angeles County. The program will provide housing placement referrals, health literacy and navigation, employment readiness, and crisis support through an integrated team of outreach workers, peer navigators, and case managers.

What is your understanding of the issue that you are seeking to address?
Los Angeles County faces an urgent crisis, with the Mayor’s declaration of a state of emergency underscoring the severity of unsheltered homelessness. OFMP, Inc. (Omnipresent Families Motivating Prosperity) is uniquely positioned to respond to this challenge through its culturally responsive, trauma-informed, and multi-generational approach. Our organization bridges critical service gaps by meeting clients at the intersection of housing instability, mental health, food insecurity, and emergency preparedness. OFMP provides holistic wraparound support that addresses the interconnected root causes of homelessness. As a trusted community partner with over two decades of experience, we are expanding our reach to serve overlooked subgroups within the unsheltered population, including seniors, youth, and families. Our model leverages existing partnerships and resources to deliver comprehensive, client-centered care that promotes lasting stability, well-being, and pathways out of homelessness.
Describe the project, program, or initiative this grant will support to address the issue.
OFMP, Inc. is expanding its role as a connector and resource navigator to deliver targeted, wraparound services that address critical gaps faced by under-resourced and underrepresented populations experiencing or at risk of homelessness in Los Angeles County. With grant support, OFMP will enhance its outreach, engagement, and case management capacity to provide direct housing placement assistance, health literacy education, and navigation to medical and behavioral health services. The program will also offer employment readiness and job placement support to help individuals achieve long-term stability. By expanding our team of culturally competent case managers and outreach specialists, OFMP will meet clients where they are—physically and emotionally—and ensure they are linked to essential, life-sustaining services. This holistic approach will reduce barriers, empower vulnerable subgroups, and promote equitable access to housing, healthcare, and employment opportunities throughout the County.

If OFMP’s initiative is successful, Los Angeles County will see a measurable reduction in service fragmentation, homelessness recidivism, and access inequities among its most vulnerable populations. Our vision is a coordinated ecosystem where underrepresented residents—seniors, youth, families, and individuals—receive seamless access to housing, healthcare, and employment services through culturally responsive, trauma-informed support. By leveraging collaborative partner resources and expanding outreach and case management capacity, OFMP will transform disconnected systems into a holistic, client-centered network of care. Success will be reflected in increased housing placements, improved health literacy and navigation, and higher rates of sustained employment. As OFMP bridges service gaps and empowers residents with stability and self-sufficiency tools, communities across Los Angeles will experience improved public health, economic resilience, and strengthened neighborhood cohesion.
