Summary: Safe Place for Youth supports youth, ages 12-25, to transition off the streets and into stable housing situations and employment. Trauma-informed care, positive youth development, harm reductions and low-barrier programs offered at Safe Place for Youth are seen as a first step to engage youth experiencing or at-risk of homelessness into a continuum of supportive services. Safe Place for Youth has been growing steadily and is now at a crossroads at which it seeks to dramatically accelerate growth. The organization is looking for a mission-focused, critical- thinking and enthusiastic case manager willing to proactively support youth in accessing health and wellness services.
Serve as the primary case manager for youth to access substance use support resources, mental health services, and other health and wellness resources
Set goals, create case plans and administer intakes and assessments as needed
Lead health and wellness groups
Support the development and engagement of members for the on-site recovery programming at SPY
Provide crisis intervention and conflict resolution support on-site
Participate in case conferencing with cross-department program staff
Build and manage relationships with community partners both for referrals for detox, recovery, sober living, mental health support, counseling, and additional health and wellness support.
Support health and wellness partner programming on site
Ensure youth have access to a wide variety of resources to address their health and wellness needs, including medicinal and non-medicinal treatment for substance use and mental health
Create and maintain partnerships with external agencies and organizations
Support with health and wellness field trips
Job Qualifications & Experience
Demonstrated ability to deal with conflict and crisis, utilizing a trauma-informed, harm- reduction approach.
1+ years experience in case management, counseling or related experience.
2+ years experience providing direct services to high-risk youth including counseling and advocacy.
Knowledge of Social Service resources for youth experiencing homelessness and at-risk of experiencing homelessness.
Working knowledge of trauma informed and harm reduction service models.
Must possess advanced computer skills including word-processing, database operations, spreadsheets and other software systems.
Proficient in Microsoft Office (Word,Excel) and Google Docs.
Advanced verbal and written communication skills, including organizational, detail and interpersonal skills.
