About this job

The Director of Enhanced Care Management & Community Supports (Director) will work under the Vice President of Strengthening Families & Communities to provider oversight to the Enhanced Care Management (ECM) and Community Supports (CS) program. This position is responsible for the quality of ECM & CS client care services, contract and program reporting, contract billing and reconciliation, staff selection and training, oversight of chart documentation, and development of health and care coordination plans. This position serves as the principal point of contact with managed care plans liaises with community medical and other service providers, hospitals and other institutions.

The Director of Enhanced Care Management supports and integrates the mission of Children’s Institute (CII) with the work of ECM & CS. This includes supporting the agency in its efforts to achieve its goals related to CalAIMS and applying the agency’s framework of a two-generation lens to the individuals and families enrolled in the program. The Director will support in building relationships to support intra-agency and inter-agency referrals. This person is also responsible for partnering with health and other service providers to coordinate health literacy activities for enrolled individuals and community members.

Roles & responsibilities

Program Management

•Ensures assigned programs are in compliance with all agency policies and procedures including adhering to all state and/or federal funding requirements and licensing/contractual requirements.

•Oversees reporting, billing, and quality assurance in partnership with various CII departments.

•Ensures continuous availability of clinical supervision, consultation, and support as required during and after normal business hours.

•Manages the development and implementation of comprehensive, coordinated health service plans for all covered families in assigned programs.

•Ensures that member insurance eligibility and accuracy of the Targeted Engagement •List/Member File, and timely tracking of outreach activities is confirmed monthly.

•Ensures timely, high-quality documentation of services in accordance with CII and contractual requirements including ensuring charts are audit ready at all times.

•Develops and monitors staff schedules to ensure program coverage and budgetary compliance.

•Provides input on the development of program budgets; reviews and approves payment of expenses and ensuring compliance with approved budgets. Ensures resources are sufficient to support assigned programs.

•Manages to ensure fiscally viable programming and services plans to ensure effective quality clinical service delivery in alignment with CII’s service philosophy.

•Manages collection of process and outcome data and utilizes the data to improve program procedures, identify needs, and highlight program successes.

Organizational Leadership

•Ensures all staff are provided with individualized orientation plans, training plans, direct field observations and performance evaluations.

•Provides direct supervision to program supervisors and direct service staff. Is responsible for providing leadership development to all program staff and partners with the Vice President to support and provide leadership for the entire region.

•Facilitates and participates in case conferences and team meetings to monitor individual needs.

•Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

•Works collaboratively with management team to integrate cross program activities and functions.

Communications & External Relationships

•Supports with ongoing communication and certification procedures with managed care providers and other contractual partners.

•Participates in regular meetings with managed care program, other contract liaisons, and community partners.

•Seeks opportunities to build strong relationships with health care providers, local and community organizations, government agencies to support enrolled members.

Qualifications

•3 to 5 years of professional level outpatient medical, community health service delivery, behavioral practice management, or related healthcare experience.

•5 to 7 years of managerial, supervisory experience.

•Bachelor’s degree in Healthcare Administration, Nursing, or a related field, and a minimum of seven years of managerial supervisory experience. Master’s degree preferred.

•Possesses a valid driver's license and state-required auto insurance.

•Spanish / English bilingual preferred.

•Experience working cooperatively and collaboratively with a wide variety of external partners.

•Experience developing, measuring, and evaluating program through effective metrics.

•Maintains patient privacy, including protecting the confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities.

•Sensitivity to service population’s cultural and socioeconomic characteristics.

•Excellent verbal and written communication skills.

•Understands CII’s basic philosophy and participates fully in carrying out its mission.
