A Place Called Home is seeking a resourceful Case Manager II
Under the direction of the Director of Counseling Services, the Case Manager II develops and implements Individual Program Plans for youth. The Case Manager II coordinates with Case Manager I and is responsible for connecting youth and family members to resources, ensuring case plans are met, and facilitating groups by performing all essential duties and responsibilities.

FLSA Status/Hours: Non-Exempt/40 hours
Pay Rate: $15.00-17.00 per hour
ESSENTIAL DUTIES AND RESPONSIBILTIES
To perform this important job successfully, the Case Manager II must be able to fulfill all of the elements of the Job Description to the highest standard, and be able to work well with colleagues in a high energy, collaborative environment with open communications.
