A Place Called Home is seeking a dedicated College Pathways Coordinator

Under the direction of the Associate Director of Teen and Young Adult Services, the College Pathways Coordinator is responsible for developing a work and/or college plan with college age members; collaborates/facilitates career and college fairs; coordinates and facilitates workshops and trainings to enhance the growth of our youth transitioning to and through college and into the work world; The College Pathways Coordinator will also identify and respond to young adult members not on a path to higher education/vocational training, in need of individualized support or in various stages of crisis that prevent or impede their progress towards achieving healthy lives that may include higher education and/or a fulfilling vocation. This position combines program support and academic guidance with dynamic case management, and the coordination of internal and external referrals/resources.

Reports to: Associate Director of Teen and Young Adult Services

FLSA Status: Non-Exempt/40 hours per week

Hourly Range: $14.00-16.00 DOE

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Adhere to the highest ethical and professional standards and values at all times
  • Manage a caseload of up to 50 scholarship recipients and create individualized plans and monitor their progress, during regular quarter/semester check-ins
  • Assist students in developing individual 2-4 year college academic and career plans and provide academic resources and referrals to support their academic and career goals
  • Facilitate meetings with parents to ensure college pathways objectives for scholars are achieved
  • Plan and coordinate quarterly professional development trainings and workshops for scholars
  • Work closely with tutors to assist scholars who need additional academic support
  • Assist with the coordination of Financial Aid workshops for members and parents
  • In collaboration with the High school Pathways coordinator, coordinate with the on-site educational program in order to support members in transition in applying for college or vocational training programs
  • Assist in coordinating group workshops for scholars geared towards successful college planning, skill development, exposure to career training and opportunities and post graduation plans
  • Maintain accurate records and case notes for scholars on caseload
  • Liaison with Counseling Department on a weekly basis by attending weekly Case Management meetings
  • Utilize Family Wraparound model and ensure services are consistent in delivery with overall services (Counseling Department)
  • Serve as an advisor and support the Scholars Leadership Committee (SSLC), a scholar led leadership group, in planning social events, reviewing policies, assisting with the APCH developments and special events
  • Develop relationships with external organizations and community partners in order to expand available resources and opportunities to members
  • Prepare and submit monthly reports on students’ progress and participation of those that have received individualized plans, referrals and counseling
  • Attend all required program meetings (Department, Check-in with Supervisor, etc.)
  • Prepare and submit all paperwork in a timely manner (Funding reports, Monthly reports, assessments, etc.)
  • Lead and contribute to ongoing development of program elements and policies
  • Assist with the preparation and facilitation of the Scholarship Committee meetings and presentations
  • Serve as a mentor for members within the Bridge to the Future Department
  • Supervise members within the program
  • Coordinate and facilitate college and career related tours, field trips and guest speakers
  • Assist with chaperoning field trips, planning and executing scholarship events
  • Assist with applications and reports for grants including documentation of case studies
  • Perform other tasks as assigned

Education/Training/Experience

  • B.A. Degree in Social Services or similar field
  • Minimum 2 years experience working with youth (ages 14 – 24)
     

Physical and Mental Demands

  • Able to work in chaotic and high noise level environment typical of a youth center
  • Walking, standing required on daily basis
  • Sitting and use of hands/vision for computer work for extended periods of time
  • Lift up to 25 pounds occasionally
  • Able to communicate and convey accurate information
  • Type 45WPM
